IMPORTANT INFORMATION – DEBIT CARD SUSPENSION
DATE: March 2024
TO: IBEW Local 332 Inside Wireman HRA Participants
FROM: United Administrative Services
RE: HRA Debit Card Suspension FINAL NOTICE
As you know, the HRA benefit under the IBEW Local 332 Health and Welfare Plan is an IRS regulated benefit. The IRS guidelines require all participants to submit itemized statements showing that the reimbursed expense is an eligible health care expense (as defined by the IRS).
The Plan sends participants at least three (3) notifications for each transaction requiring documentation. We have even made additional attempts to contact participants by phone who have outstanding transactions. There are currently 1,054 participants with outstanding transactions because they have not submitted the required documentation. We have only received 46 responses.
On May 1, 2024, any participant who still has outstanding debit card transactions will have their HRA debit card suspended until ALL outstanding transactions have been cleared.
Please view your HRA account online at https://uasbpppt.lh1ondemand.com to see if you have any outstanding transactions or you can contact Jo-Ann Rashid. Jo-Ann is at the 332 union hall Monday thru Friday from 1:30 p.m. to 5:00 p.m.; or you can reach her at (408) 288-4493, or (650) 393-0180. You can also email her at jrashid@uastpa.com.
Going forward, we will continue to notify you by email or letter when documentation is required. Failure to do so within 60 days from the transaction date will result in suspension of your HRA debit card.
